Private Resomation Agreement
The 'Private Resomation Agreement' must be filled and signed by the Nearest Next Of Kin. It must be completed for each individual resomation of an adult, infant, stillbirth baby and of a body organ.
Resomation Forms For Funeral Directors
Resomation forms must be completed for each individual resomation of an adult, infant, stillbirth baby and of a body organ. They must confirm the details of the deceased, the authorisation by the correct individual and the circumstances surrounding the cause(s) of death.
Northern Ireland and United Kingdom
​For resomations coming from Northern Ireland and United Kingdom, Forms A, B and C or D need to be completed along with Verification of Infectious Disease Status Form and the Out of Country Certificate. Furthermore, resomations from these countries require additional documentation such as Permission To Cremate and Death Certificate.
